HLF2026 Discovery Sessions

The forum starts off with 3 concurrent Discovery Sessions, running from 10:00am – 12:00pm. Together, they feature the 11 projects awarded under Window 1 of the MSF PATH Fund (Partnerships to Tackle Homelessness), grouped into 3 themes.

Theme 1: Tailored Outreach and Support

How tailored, relationship-first outreach and support reaches persons whom mainstream services often miss — from youth in shelters to rough sleepers on the streets.

S3P Specialised Support Pathway for Youth

Youth Guidance Outreach Services & New Hope Community Services

This project provides homeless youths in transitional shelters with dedicated, youth-centred support to better meet their developmental and social needs, through a partnership between YGOS and New Hope Transitional Shelter.
 
Youths aged 18–25 who are living away from home due to unstable family environments are guided through a three-stage transitional living pathway — from shelter preparation, through targeted developmental work, to independent living support.
 
Support includes mentoring, family mediation, independent living skills training, and vocational guidance, with the goal of transitioning youths into education, employment, or training as a stepping stone to financial stability and independent living.

Galilee - Drop-In Centre for Rough Sleepers

Catholic Welfare Services 

This project opens a welcoming, non-judgmental drop-in space where hard-to-reach rough sleepers can access meals, showers, and counselling support at their own pace.

Community Gatherings

Homeless Hearts of Singapore 

This project builds trusting relationships with rough sleepers who are reluctant to seek formal assistance, by organising community gatherings in selected neighbourhoods where rough sleepers come together with volunteers who journey alongside them and connect them to support services.

Transformative Retreats for Residents in Rental Housing: Co-Design, Pilot and Playbook

NUS IPS & New Hope Community Services

This project pilots a structured retreat programme for persons who have experienced homelessness, to overcome psychological and relational barriers, build confidence, and sustain their transition into stable housing.
 
Participants will take part in group sessions focused on reflection, rebuilding trust, and identifying their personal strengths and aspirations.
 
Learnings from the pilot will be consolidated into a playbook documenting how the model can be designed, implemented, and adapted for this target group in Singapore's context.

Upper Room - Rough Sleeping Anonymous

Catholic Welfare Services 

This project provides group counselling sessions — facilitated by trained counsellors — to help rough sleepers and former rough sleepers address the emotional and behavioural factors that may lead them to return to rough sleeping. The sessions aim to help participants overcome psychological and relational barriers, build confidence and resilience, and navigate the stigma associated with homelessness as they work towards stable housing.

Holistic Empowerment, Adjustment and Life Skills (HEAL)

SowCare

This project provides rough sleepers with a structured programme covering mental wellness, life skills, financial planning, and family counselling to help them achieve long-term stability and reintegrate into the community.
 
The programme addresses emotional, relational, behavioural, and practical barriers to reintegration through mentorship, interpersonal skills development, and family reconciliation where appropriate.

Safe Sound Sleeping Retreat Places - From Shelter to Sanctuary

Bless Community Services 

This project pilots the transformation of a Safe Sound Sleeping Place (S3P) into a Retreat Place (S3RP), providing shelter residents with wraparound support to help them transition into and sustain stable housing. The transformation plan is structured around the three domains: People (Equipped Carers and Companions), Place (From Sleeping Space to Restful Space), and Programme (Recovery and Empowerment). 

Residents will receive counselling, life coaching and also go through a structured programme focused on building life skills and social connections, supported by trained staff who deliver trauma-informed, person-centred care. 

The goal is to help residents move beyond immediate shelter needs to personal stability and well-being, social reintegration, and long-term housing stability .

Solutions that help households stay housed for the long term, from neighbourhood coordination to employment pathways.

Local Area Coordination at Woodleigh (JSS-OR)

This project pilots a Local Area Coordination (LAC) approach for newly-housed rough sleepers under the Joint Singles Scheme Operator-Run (JSS-OR) at Woodleigh Link, helping them familiarise themselves with their new neighbourhoods by linking them to key community networks such as social services, neighbours, local businesses, and community programmes.
 
Volunteers will be trained as "Local Community Coordinators" who identify relevant community resources and assets to introduce residents to, empowering them to access support close to where they live.
 
The project aims to assess whether LAC can better sustain residents' exits from homelessness and support their adaptation to independent living.

Good News Community Services

Common Ground - JSS Matching Programme

Good News Community Services 

This project pilots structured Joint Singles Scheme (JSS) matching sessions to connect single rough sleepers with compatible co-tenants, helping them access housing more quickly while reducing the risk of shared living arrangements breaking down.
 
Applicants are pre-screened for compatibility in daily habits and routines, and guided through sessions designed to build the skills needed to live successfully with a co-tenant.
 
The project aims to reduce co-tenant conflict as a driver of return to rough sleeping, and support more sustainable housing outcomes.

Hope Initiative Alliance

MAPS - Micro-employment and Apprenticeship Pathways to Stability

Hope Initiative Alliance

This project pilots an employment pathway for rough sleepers and shelter residents, providing tailored opportunities from apprenticeships to micro-jobs to build work readiness and achieve greater housing stability.
 
Participants are supported through work-readiness preparation, employer matching, mentorship, and practical assistance as they transition into employment, with the aim of improving their long-term prospects and securing sustainable livelihoods.

New Hope Community Services

From Tenancy to Sustainability: A Risk-Informed Community Work Approach in Social Housing

New Hope Community Services

This project develops a tool to identify early signs that a person may be at risk of homelessness, enabling timely support to help them remain stably housed.
 
Selected tenants in an HDB rental housing setting will undergo a structured assessment covering their physical and psychological well-being, social connections, living environment, and sense of empowerment and purpose, with targeted interventions provided based on identified risks.
 
The goal is to identify tenancy breakdown risks early and deliver the right support at the right time, to keep residents stably housed.
